The following is a list of documents that we require you to read and/or sign when enrolling in our classes or diet coaching.

There are three ways you can sign and return the documents:

  • Manually:
    1. Click the link to open the file and click the "Print" icon in the upper right corner.
    2. Fill out and sign the form.
    3. Scan the form and email it to or mail it to

  • Digitally (PC/Mac):
    1. Click to open the file and click the "Save" icon in the upper right corner or right click the link and click "Save Link As".
    2. Open the document in Adobe Acrobat Reader DC and follow this guide to fill out and sign the form electronically.

      NOTE: Once you have added your signature, Adobe Reader will lock the document and will no longer allow you to edit the form.

  • Digitally (Mobile):
    1. Download the Adobe Fill & Sign app for your device (free for iOS and Android).
    2. (Optional) Fill in your profile for quicker form filling.
    3. On our website, copy the link for the document you want to sign.
    4. In Adobe Fill & Sign, tap "Select a form to fill out." Tap "Web URL," paste the link in the box, then tap "Fill Form."
    5. Once the form opens, tap anywhere in the form to add text fields and fill in your information. You can paste information from your profile for convenience.
    6. Tap the signature line to add a text field, then tap the signature button at the bottom. You will be prompted to add a signature if you don't already have one saved (you can draw a signature or use a photo). You can move and resize your signature as needed. Tap anywhere on the form when done.
